Full Job Description

Following an internal promotion, we have a vacancy for a Lead Pathways Co-ordinator. This role oversees the Pathways team who undertake tasks essential to the smooth and effective provision of quality care, providing an efficient level of service for patients and their families. The role will ensure the team works within the priorities and expectations set out by the Outpatient Management team and regularly report on team progress and output. It is essential the post holder exercises people managements skills, initiative and judgement whilst maintaining a high level of confidentiality and diplomacy at all times. The team lead is the person to whom complex patients are escalated and will also make decisions where appropriate. No candidate will meet every single desired qualification. If your experience looks a little different from what we've identified and you think you can bring value to the role, we'd love to learn more about you., Organisation, prioritisation, communication, IT skills and People Management. Attention to detail will also be crucial in this role as you will be ensuring we have the correct documentation, results and images on patient records for clinical decisions to be made and patient care progressed. Full training will be given on the patient record system and how to complete the role.
