Full Job Description

We are recruiting for a Lead Physical Health Nurse to join our Mental Health teams at the Whiteleaf Centre in Aylesbury where you will be working with our Adult and Older Adult Inpatient services to deliver high quality care. This is a great opportunity for a registered General Nurse/Dual Qualified Nurse (RMN/RGN) with excellent experience to support teams deliver safe and excellent physical health care for patients with complex mental health problems. You'll be instrumental in driving the physical health agenda across the mental health directorate, facilitate training and work collaboratively with services to provide good holistic care. You'll be a positive role model to our teams, working closely within the Multi-Disciplinary Team and be integral in decision making that improves physical health outcomes for patients. You'll also be involved in Quality Improvement work across the wards to improve overall service provision. This is a full-time role based at the Whiteleaf Centre in Aylesbury Buckinghamshire.,

  • Advise and contribute to the delivery of high-quality nursing care for patients with physical health issues within the A&OA wards.
  • Encourage and develop innovative practice that will improve the overall health and well-being of individuals with mental health problems who use the Trust services.
  • Lead on the development and delivery of high-quality nursing care plans for patients with Serious Mental Illness which advise on physical healthcare including smoking cessation, diet, weight gain, exercise, lifestyle, sexual health, and side effect monitoring.
  • Develop, co-ordinate and contribute to the delivery of physical health and well-being training programmes and educational materials for staff.
  • Partake and lead on audits and research involving the physical wellbeing of patients throughout the service.
  • Lead on developing and implementing a health promotion strategy within the service. Identify appropriate tools to measure the effectiveness of well-being support and health promotion programmes.
  • Provide leadership to physical health leads within each ward and community services.
