1. To be an integral part in the provision of an effective and high-quality community stroke and neuro rehab service, liaising closely with therapists and nurses from University Hospital (UHCW), community therapy pathway and community discipline specific services. 2. To follow defined stroke and neurological care pathways and other relevant rehabilitation pathways as required. 3. To utilise a comprehensive range of evidenced based specialist assessments and treatment to patients, participate in triage, to determine whether the referral is appropriate and establish any clinical risk. 4. Through highly specialist clinical reasoning and practical application determine specific therapeutic needs, taking into account holistic and interdisciplinary requirements. 5. To effectively manage and prioritise a large caseload of patients with complex conditions based on the nature, complexity and urgency of each individual case. 6.
