Full Job Description
- Work in partnership with other agencies and stakeholders and where needed arrange and chair multi-disciplinary and best interest meetings
- Possess a Comprehensive knowledge and understanding of the legislation that underpins Adult Care.
- Safeguarding knowledge must be up to date, and you should be fully familiar with your own and team's responsibilities.
- Put systems and processes in place to monitor and review your team are on track with their key performance indicators

CV's will not be taken into consideration. About the Role You will manage a team of qualified and unqualified Practitioners. Your team will have an allocated case load which includes complex and high-risk cases. Your team will carry out strength based Adult Needs Assessments and Reviews, working with service users and their families to maximise independence, assessing, and managing identified risks, taking into consideration any informal support provided. We work in accordance with the Care Act 2014, we are creative to ensure needs are met. For example, support in the community, voluntary services, or use of equipment such as Telecare. We must ensure the public purse is managed wisely.

- Ensure your team are working within the financial constraints of our service budget, arranging strength-based cost-effective packages of support, In accordance with Part 7 of the Immigration Act 2016 (Fluency Duty), the ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
The post will be based at our Boston office which is: Unit 1 Carlton Centre, Carlton Road, Boston PE21 8LN We celebrate our Teams achievements and share our experiences.
