Full Job Description

+ The post holder will work with the Clinical Lead and other key stakeholders within the Organisation to achieve the strategic objectives across the Trust relating to Palliative/End of Life Care.
+ The post holder will develop and maintain an integrated specialist palliative care team, promoting cross organisation/professional boundary working.
+ The post holder will be required to maintain their clinical expert practice, whilst promoting and creating a wider vision for palliative and EoLC.
+ The post holder will champion the quality strategy and uphold the values and standards of the nursing / AHP profession.
+ The Post holder will ensure Specialist Palliative Care and generalist services across the trust are responsive to the needs of patients with palliative and EoLC needs, in line with national Ambitions for Palliative and EoLC, and NICE guidance. This will involve working with key providers, local commissioners and regional strategy groups.
+ The post holder will develop specialist palliative care workforce to be responsive to need and to support quality improvement across the trust.
+ The post holder will support staff learning and development across the trust in palliative and EoLC to provide patient and family centred care., · To be an integral part of the Trust's senior nursing, midwifery and AHP structure providing strategic leadership for palliative and EoLC. Leading a Trust wide palliative and EoLC strategy and liaising and building strong working relationships with key stake holders.
· To lead on the delivery of national Ambitions for Palliative and EoLC, NICE guidance and CQC requirements for EoLC.
· To lead on the delivery of quality improvement initiatives for palliative and EoLC including advance care planning, priorities for care in the last days of life, and accreditation for the EoLC quality standards.
· Establish and maintain the effective management of the Specialist Palliative Care Service within the Integrated Care Division.
· Act as a role model, maintaining a visible and credible profile within the Division, and as part of the wider Trust leadership structure.
· To provide professional advice and leadership for the development, implementation and monitoring of palliative and end of life care strategies within the Trust that deliver a high quality, patient centred service.
· In conjunction with the Divisional Nurse / Therapy Director and the senior staff in the Division, to strengthen leadership in order to continuously improve patient safety and patient experience.
· Participate in the Professional Cover Rota., · To provide effective line management and professional leadership with regards palliative and EoL care, and to provide line management for the Specialist Palliative Care workforce.
· To identify and lead changes in practice and service delivery in accordance with evidence-based standards and policy, challenging traditional boundaries and current thinking and overcoming resistance and conflict at Trust level.
· Maintain a high profile within the Trust and externally, providing a strong contribution to the achievement of organisational goals.
· Provide support and supervision for senior nursing staff working towards a higher level of practice.
· Communicate regionally, nationally and internationally on area of specialist knowledge.
· To work collaboratively and strategically with representatives both internally (community and acute) and across Greater Manchester around the co-ordination and delivery of palliative and end of life care services.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90 This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
