The role is for a Lead Practitioner to manage and develop a high performing operational Adult Frailty and Long-term conditions team with a focus on improving outcomes for customers. The team is committed to providing an excellent service promoting an integrated and strengths based, person-centred approach. As a Lead Practitioner with LCC you will have the opportunity to lead and develop your dedicated team of social care staff, both qualified and unqualified. Collaborating equally to ensure continued learning, innovation, support and delivery of a high-quality service. You will be supporting and encouraging your team to have the confidence to work in a strengths-based way. The aim of the team is to work hand in hand with health colleagues and third sector staff to ensure that the people we support in the community get a streamlined service. We think creatively when planning support packages, utilising "outside of the box" thinking and solutions to ensure individual care planning empowers the service user. About The Team We are a team of Community Care Officers and Social Workers providing services within the Gainsborough area. About You It goes without saying that you must be a professional Social Worker, Nurse or Occupational Therapist who has the significant skills and experience to undertake this role.
