Full Job Description

Our adult and paediatric liver service is one of the largest of its kind in Europe and is at the forefront of research and development in the management of children and adults with liver disease and transplantation. As a specialist tertiary centre we care for patients from all over the UK and Europe with a dynamic and committed team of staff. We also have strong links with national and international liver transplant programmes.
There is now an exciting opportunity to lead a dynamic specialist team developing a unique service providing specialist, individual care to patients and their carers throughout the transplant process.
This is a challenging and rewarding post where you will be given every opportunity to develop your potential. You will be expected to provide leadership in the management of all aspects of transplantation (adult & paediatric) working in partnership with the clinical leads, clinical director, head of nursing, transplant service manager, external health care providers and regulatory bodies.
You will liaise with regulatory bodies including NHS blood & Transplant, and the Human Tissue Authority. We are looking for a clinical expert who can lead a team as well as contribute as a role model in delivering high quality patient care.
Occasional on-call work may be necessary and participation in providing cover for the living donor transplant coordinator., The post holder will have an advanced level of knowledge and expertise and provide leadership in the management of all aspects of transplantation (adult & paediatric) working in partnership with the clinical leads and clinical director.
The post holder will provide clear focus for clinical leadership within the service. They will be highly visible and responsible for ensuring the delivery of high quality of care.
To be responsible for writing, implementing and evaluating clinical guidelines, promoting practice developments within the clinical governance framework to ensure local and national objectives are achieved.
They will be responsible for coordinating care, monitoring the quality of care and ensuring the needs of pre transplant, waiting list and post-transplant patients are always at the centre of that care. A proportion of time will be spent working clinically.
They will provide professional leadership and management to the defined team of transplant co-ordinators, specialist social worker and alcohol & substance misuse CNS demonstrating a supportive and flexible leadership approach and a proactive, responsible attitude so as to promote confidence in the team and the service. You will be required to explore current practice, identifying areas for development, and plan the appropriate change using research and experience to evaluate the outcomes., + To lead on the operational and strategic development of the specialist transplant coordinator service.
+ Ensure that staff are appropriately trained to deliver safe, high quality patient care for patients in all stages of the transplant process.
+ To review and monitor service performance through participation and initiation of clinical audit and developing policies and protocols to improve patient care.
+ To initiate and participate in the management of change to improve standards and communication links.
+ Monitor and manage risk, ensure risks and complaints polices are adhered to undertaking assessments in local areas of responsibility.
+ To be actively involved in NHS England Liver Transplantation Service Peer Reviews and Human Tissue Authority Audit visits as necessary. Contributing information and experiential evidence to support achieving standards therein.
+ Ensure NORS standards are adhered to and protocols are updated as necessary. Addressing / investigating all clinical incidents received from or submitted to NHSBT in a timely manner, in collaboration with the clinical lead for NORS. Ensuring shared learning and the implementation of any recommendations.
+ To be responsible for managing a team of Transplant Coordinators, Clinical Nurse Specialist, Social Worker, Audit nurse and administrator ensuring annual appraisals are performed, mandatory training is maintained and that sickness, study leave and other absence is monitored and dealt with appropriately.
+ To manage the allocated nursing budget, ensuring effective cover of the 24 hour transplant coordinators on-call rota, reviewing practice and financial implications, in conjunction with the head of nursing.
+ Maintain contemporaneous and accurate reports collaboratively with the service manager submitting relevant statistics, reports and activity data as requested to care group, NHSBT, NHS England. Evaluating the service in terms of clinical effectiveness , clinical excellence and highlighting current issues impacting the service
+ To manage and maintain the Team's development of influential relationships with surgeons, anaesthetists, physicians, nursing staff and other members of the MDT within and outside the boundaries of King's College Hospital.
+ To ensure the team work within the boundaries of NHSBT and NHS England contracts, EU Directive, HTA, and LAG polices and guidelines.
+ Attend care group meetings as requested
+ Facilitate and co-chair with the relevant lead bi - monthly NORS, Transplant audit meetings ensuring all necessary actions are implemented and disseminated to the appropriate staff members.
+ Facilitate and co-chair with the relevant lead bi - monthly meetings with Regional SN-OD team leaders.
+ Joint management of patient / relative on site accommodation, ensuring functionality upkeep and invoicing.
People Management and Clinical Performance
+ Provide an expert liver transplant specialist nursing service and to practice at an advanced level consistent with required competencies
+ Expectation to work clinically for a proportion of time, split to be decided after a review period
+ To provide professional guidance, support and leadership to all nursing staff within the unit, demonstrating a responsible attitude in order to promote confidence in the service.
+ To be an effective role model and able to represent the Care Group organisationally and professionally.
+ To be highly visible, accessible and approachable to nursing staff, patients and public so that open and honest communication channels are created and effective assessments of care delivery can be undertaken.
+ To provide direct support to the wards in areas related to own clinical expertise or guide staff to relevant personnel for clinical support.
+ To be responsible for reporting complaints and resolving them, or assisting with resolution in a speedy and courteous manner working with the Transplant service manager.
+ To investigate accidents and incidents and implement corrective action where appropriate, in consultation with the Head of Nursing.
+ To coordinate a seamless service through the development of enhanced multi disciplinary team processes and communications.
+ To work with key stakeholders to establish nurse led outpatient assessment clinic and ongoing post-transplant short and long-term follow up.
+ To review weekly the status of patients on the transplant waiting list to ensure the accuracy of the waiting list held by NHSBT.
+ To liaise with referring consultants, community, patients and relatives to obtain feedback to help develop the service for patients, carers and health care professionals.
+ To actively involve service users in providing feedback of their experience of the current service and give suggestions for its improvements and development.
+ To provide cross cover for the living donor transplant coordinator in relation to the living donation liver transplant (adult/paediatric, adult/adult) programme, covering annual leave and a proportion of on call weekends.
+ Establish and maintain a peer network of support, information and learning from colleagues at a national and international level to exchange and enhance knowledge and expertise.
+ Lead, coach and manage the performance of the team in line with good people management practices. Ensuring excellence is recognised and under performance is addressed.
+ Participate in regular performance appraisal meetings and ensure each member of the team has a clear set of objectives and development plans.
+ Ensure the team is compliance with all statutory, mandatory training together with any professional training requirements, ensuring they are up to date and fully compliant.
+ Manage team absences including sickness in line with Trust policy ensuring the appropriate return to work meetings occur, e-roster is updated and productivity is at keep to the highest possible level.
+ Identify and fill any vacancies that arise within the team in line with the Trust's recruitment policy and process.
+ Identify talent and support the internal talent management process in order attract and retain and succession plan for your people.
+ Ensure overall wellbeing of the team is maintained. Continuously support in improving the morale of the team and implementing a culture of zero-tolerance for bullying and harassment.
General and On - Call responsibilities
+ Participation in the provision of an on call weekend service for Living Donation in the setting of Acute Liver Failure.
+ To manage and supervise an appropriate on-call service, evaluating it's effectiveness and efficiency on a regular basis ensuring all potential liver/ liver & kidney /small bowel / islet / hepatocyte offers are facilitated 24 hours a day, 365 days a year.
+ Provide an appropriate training programme to ensure that transplant coordinators can achieve the required competencies to gain independence in regards to the on call service., + To promote and educate patients, health care professionals and the general public about the benefits of transplantation/organ donation.
+ To communicate complex information to potential recipients and their carers using a high level of communication skills to facilitate understanding, acceptance and reassurance.
+ To contribute to the development of nursing practice within the team and act as a clinical supervisor.
+ Undertake all required audits / contract reports to ensure appropriate information is readily available within the trust and to external agencies, NHS England, NHSBT, HTA and LAG in conjunction with the transplant service manager.
+ Ensure all staffs competencies are signed off appropriately

+ Achieve the range of advanced core competencies as required within the Trust and Care Group necessary for a recipient transplant coordinator to act as an autonomous practitioner, adapting to changes of working practise in a rapidly developing speciality.
