Full Job Description

An exciting opportunity has arisen for an experienced Learning Disability nurse within the CHC/CC/Complex Care team within Cheshire & Wirral Partnership NHS Foundation Trust. The successful applicant will have a working knowledge of Mental Capacity Act, Safeguarding, Deprivation of Liberty Safeguards, experience of undertaking assessments, case and risk management and personalised care agenda will be essential for this role.

The post-holder will undertake various functions relating to the National Framework for NHS Continuing Healthcare which will include undertaking health needs assessments, facilitation of multi-disciplinary assessments, reviews, supporting individuals with case management and staff leadership and management.

We require a motivated and experienced registered learning disability nurse who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) including the promotion and review of Personal Health Budgets.

Access to transport for work purposes is essential.

The post is part time 30 hours per week., The role will include meeting individual on-going health care needs for learning disability clients, in conjunction with the Continuing Health Care Teams and coordinating the process of assessing for NHS funding.

To provide specialist LD nurse case management on behalf of ICB clients with regards to the commissioning, planning and coordination of bespoke care packages for learning disabilities service users with complex health needs across Wirral.

To manage a complex and varied case load of people with learning disabilities and have the responsibility for coordinating the process of identifying and securing appropriate health funding, either through CHC or joint funding arrangements with the relevant Local Authority; with support from the Clinical leads .

To be part of the development and monitoring of effective working relationships and processes between other NHS Trusts, General Practitioners, Primary Health Care Teams, Local Authority, Independent Providers and Voluntary Services.

To contribute to the ongoing development of effective assessment, case management and review systems for service users with a Learning Disability.

Advise and support CHC Operational teams with regard to service users with learning disabilities.
