Full Job Description

The post holder will work as an autonomous practitioner, using specialist clinical skills, liaising effectively with all stakeholders and delivering training to care home staff and the network.

The post holder will be an integral member of a diverse multidisciplinary service which provides expert, caring and holistic support.

You will have access to professional supervision from the Learning Disability and will be actively encouraged to participate in continuing professional development activities commensurate with your post. There is excellent staff support, regular appraisal and CPD.

To work collaboratively with care homes in Wandsworth to meet the health needs of people with learning disabilities,reducing or preventing the need for additional health services.

To focus on implementing the NHS England Enhanced Health in Care Home (EHCH) framework and utilise learning from the locally commissioned report for Wandsworth on Covid Recovery (2022).

To focus on identifying and addressing health inequalities.

To work as an autonomous practitioner, using specialist clinical skills, liaising effectively with all stakeholders and delivering training to care home staff and the network.

An exciting opportunity has arisen for a highly motivated, innovative and experienced Nurse to join the Wandsworth Learning Disability Healthcare Team.

The successful candidate will have a substantial experience in working with people with intellectual disabilities with complex health needs; including epilepsy, dementia, challenging behaviour and mental health.
