Full Job Description

People living in care homes should expect the same level of support as those living in their own homes. The national framework for Enhanced Health in Care Homes (EHCH) provides steps to achieve this, through collaborative working between health, social care, voluntary, community and care home partners - National EHCH Framework.

The EHCH model moves away from traditional reactive models of care delivery towards proactive care, centred on the needs of individual residents, their families, and care home staff.

You will be a qualified learning disability nurse who will undertake appropriately delegated work. You will be a key member of the EHCH team and will work with the team to support person centred care plans. You will be responsible for promoting the completion of Annual Health Checks, liaise with GPs to support attendance of individuals with a learning disability at the AHC appointment and provide individual health facilitation advice where needed., The postholder will work alongside Care Co-ordinators to coordinate and communicate individual care plans with system partners and develop relationships with care homes, community teams, mental health services, palliative care services, social services, and secondary care frailty teams, to enable residents to meet their health goals and be cared for in their preferred place of care.

You will follow standard operating procedures within the limits of your defined clinical competence, and this will be reviewed regularly as the role evolves. You will work within your team and have the opportunity to escalate concerns and complexities to the multi-disciplinary team meetings.

The role will also include working with the Primary Care Network (PCN) to identify gaps within local service provision and work to reduce health inequalities within the learning disability provision. This may include supporting GP practices within the PCN to complete annual health checks for the learning disability population who do not reside within care homes.

As the role evolves it will lead to education sessions in a group setting exploring health education, screening processes and conditions within the learning disability population., 'This is an excellent opportunity for the successful individual to develop skills, knowledge and experience working with other clinical team members and across multiple PCN teams to support the health and wellbeing of our care home residents. Southport and Formby Health is a dynamic and expanding GP federation, working with Southport and Formby Primary Care Network (PCN) and other system providers to provide high quality, integrated, patient services. We are commissioned to provide the Enhanced Health in Care Homes Specification across Southport and Formby.

The role is a fixed term contract ending on 31.03.2025 and although advertised as full-time, part-time applications will be considered.
