Full Job Description

The post holder will be a registered with The HCPC and will be responsible for patients allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable.

The post holder will achieve this by providing a specialist role within the Redbridge Learning Disability CommunityHealth & Social Care Team and will demonstrate a developing level of decision-making and soenable the monitoring and implementation of standards of care.

The post holder will support their peers and team leader and be an effective team member whilst working on their own initiative. They will have a commitment to collaborative working and actively supporting and liaising with other health and social professionals and agencies.

The post holder will work with Adults from age 18.

Main duties of the job

1. To undertake the assessment, planning, implementation, evaluation and discharge of service users, with the understanding that there should be communication with senior clinicians and OTs if concerns arise.

2. To treat service users in inpatient settings and/or their own homes and other community settings, assisting and empowering service users towards independence where appropriate.

3. To assist, in conjunction with the senior clinician/ team manager, in the review and evaluation of the effectiveness of Occupational Therapy care and practice provided.

4. To work as part of a team with other therapists, nurses, social workers and any other members of the MDT.

5. To be conversant and comply with the organisation policies and clinical guidelines

6. To be responsible for providing accurate records of information required by the Trust for audit purposes.

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735) pro rata for part time staff.
