Full Job Description

A new and exciting opportunity has arisen for an enthusiastic B3 Support worker to work within our embedded community outreach team within the current existing Liaison and Diversion service. The team with support of criminal justice partners and colleagues aims to divert people away from the criminal justice system and reduce reoffending by addressing underlying social vulnerabilities and unmet needs. If you are seeking a new challenge, we encourage you to apply and be part of a new innovative team., Working within our Warwickshire Outreach team, you will support the teams practitioners in their assessment and interventions by identifying social vulnerabilities, working with partners within the third and voluntary sectors such as substance/alcohol misuse services/housing services etc to meet unmet needs/vulnerabilities and provide short term ongoing support in the community, to ensure they are able to access further support needed on discharge. You will liaise with liaison and diversion outreach practitioners , key stakeholders and partners within the criminal justice system as well as the third sector.
