Full Job Description
We have an exciting opportunity to seek a dynamic manager with experience or aspirations of clinical team management who will be excited to be part of something new and very special. We want creativity, innovative ideas and passion towards providing excellent patient care.
Do you have experience in working with learning disabilities/mental health with a level head who is aware of the pressures of a busy community environment to support and guide staff. We are looking for a strong and proven leader who can take responsibility for decisions and support junior members of the team., This role in-particular is to lead and manage our LD&A Liaison Team in Suffolk. The team is very patient-focused and highly skilled and ensures that people with a learning disability and/or autism have their health needs met by primary care services in a timely and appropriate manner. The team helps to address health inequalities that people with learning disabilities and/or autism face as part of their routine care. The team consists of qualified nursing staff and Health Facilitators and is a community-based service across East and West Suffolk and operates 9-5pm, Monday to Friday.
The post holder will also take a lead for physical health across the LD&A Specialist Pathway & lead on the development of the Dynamic Support Tool - Physical health.
