Full Job Description

We are recruiting a Peer Support Practitioner to join our Live Well Service where you will co-deliver courses designed to support people to live well with a long-term health condition and provide one-to-one peer support. This role is based on peer support principles enabling individuals to build skills and strategies and develop the hope, control and opportunities to live well with a long-term health condition. This role focuses on outcomes that matter to people as well as those who support them. You would be joining friendly, passionate colleagues including co-production coordinators, therapists including occupational therapists, dieticians, speech and language therapists, rehabilitation coaches, psychologists, physiotherapists and administrative support. This team is committed to delivering high quality person- centred care.This is a part-time role (15-20 hours) which can be spread across a working week of 08:30-16:30 Monday to Friday. The role is based at Woodland House and the

The Live Well service is looking to employ individuals with lived experience expertise to add value to the growing team. Individuals with personal experience of living with a long-term health condition and navigating services to support their needs are encouraged to apply. Suitable applicants must be in a positive position of wellbeing and motivated to take on a rewarding new opportunity. Successful candidates will use their lived experience expertise to co-deliver training programmes and courses, and to support the engagement of others who may find themselves in familiar life circumstances. Peer support practitioners complement the Live Well team's expertise, with their own lived experience expertise, insight and potential to support others. You will need to be able to share your journey to motivate and encourage others to achieve outcomes that matter to them.o To work within the spirit of co-production and co deliveryo Establish a trusting and supportive role with those

This would mean talking and listening for extended periods of time.o Highly developed skills in peer support and knowledge of the values and principles of peer supporto Team workingThe ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply
