Full Job Description
This is a new and exciting post within the Department of Medicine for the Elderly (DME)/Outliers and the successful candidate will work with existing substantive Consultants to provide a comprehensive service for Elderly patients and Medical outliers with plans to span across both the acute and community setting. The post holder will work across multiple wards and medical outlying areas. The post holder will support the delivery of an Acute Frailty specialist service as part of the delivery of the Needs Based Care Model which has been implemented across the Trust. This ensures that people arriving to hospital as an emergency attendance are reviewed by the most appropriate specialist to meet their needs.
This is an opportunity for an enthusiastic and energetic geriatrician to play a key role across multi-disciplinary teams focussed on further developing responsiveness and capacity in a range of services for older people and outlying medical patients with complex needs in the Luton & Dunstable area both within the hospital and the wider community.
The on-call physician in medicine for the elderly is the site medical co-coordinator in the event of a major disaster.The post holder will contribute to the collective aim of Bedfordshire Hospitals which is to enable older people to live as independently as possible, empowering them to take control of their condition, in order to prevent avoidable deterioration and reduce the need for acute hospital intervention., Maintenance of the highest clinical standards in the management of all patients presenting to the Hospital. This includes optimising safe and timely discharge of patients and supporting patient flow.
To share, with colleagues in medicine and medicine for the elderly for the management of all complex patients referred to the Trust. In addition, the post-holders will have the following general medical responsibilities: on a rota with colleagues, senior medical input for frail elderly into the Emergency Assessment Unit and Emergency Department.
To work collaboratively with partner organisations to actively support the further development of integrated care, specialist guidance to community MDT teams, avoid unnecessary hospital admissions while managing patients via rapid access clinic, direct admissions to frailty unit for crisis interventions without admission denial and aim short length of stay with individualized early comprehensive discharge plans.
To actively participate in both departmental and Trust matters concerning Clinical Governance and audit.
To have responsibility for and actively participate in continuing professional development (CPD).
