Long Term Conditions Nurse

As a Long Term Conditions Nurse you will be responsible for ensuring a comprehensive handover of services when patients are transferring across wings or custodial care settings. You will identify immediate health needs including, any external services or prescribing requirements that prisoners may have received e.g. prior to remand or repatriation onto a wing A key part of this role will include the facilitation of arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care.
