An exciting opportunity has arisen for an experienced long term condition clinical lead nurse to join a busy multidisciplinary teams at HMP Durham., The role delivers a high standard of care on the treatment of minor illness and minor injury and the post holder will have expertise in the management of long-term conditions such as Diabetes, COPD, asthma, heart disease and support cancer screening services. The post holder will be experienced in the delivery of Public Health England's health promotion programs such as Immunisations, women's health, and lifestyle conditions such as smoking and obesity. The long term condition clinical lead nurse will have management, leadership and development responsibilities for the healthcare team., Whether you are starting out in your healthcare journey or are an experienced healthcare professional looking for a new challenge we've got a role for you!

The post holder will practice independently and autonomously delivering care and treatment to patients at HMP Durham. The post holder will have gained a wide range of knowledge and clinical skills in a Primary care setting., The post holder will have excellent organisational and time management skills to ensure clinics and services are managed efficiently and effectively, without compromising patient care or safety.
