Full Job Description

An exciting opportunity for a Band 6, to join the respiratory lung cancer team at wye valley NHS Trust has arise. The post will be initially fixed term for 6 months, with potential this will be extended.

The post holder will work under the guidance of the Band 7 Lead Lung Cancer CNS, to provide specialist clinical advice and support across service boundaries, to act as key worker for patients with or suspected to have lung cancer., To work under the guidance of the Band 7 Lead Lung cancer CNS, to provide a high level of specialist knowledge to patients, their families and other health care professionals.

Providing pre-diagnosis support, streamlining investigations to meet patients' needs and national targets.

To work closely with Multi-Disciplinary Team (MDT) coordinator to ensure efficient & effective running of MDT meetings. Present patients to the Multi-Disciplinary Team, ensuring a holistic assessment and contributing to the diagnosis

Communicate the outcome of multi-disciplinary team (MDT) meetings to patients, providing support and information (both verbal and written) at the time of diagnosis and throughout the disease process. Ensuring continuity as patient's care moves between teams and sites and updating communications to patient journey

To act as a point of contact/support by telephone for patients and their families/carers.

Support the lung cancer pathways, ensuring implementation of the National Optimal Lung Cancer Pathway and 28 day diagnostic pathway in lung cancer. Reduce respiratory RTT and improve capacity for lung cancer surveillance

Provide expert holistic assessment and management of symptoms using both pharmacological and non-pharmacological interventions. Utilising non-medical prescribing qualification when appropriate
