An exciting opportunity has arisen at London North West Healthcare Trust for an enthusiastic, motivated and dynamic individual to join the Specialist Palliative Care team as a Macmillan End of Life Assistant Practitioner. The Trust's palliative care service is responsible for delivering high quality end of life care to patients and carers based in the acute hospital setting, so if you thrive on a challenge and feel you can make a real difference to the care of our patients, this post may be for you., The palliative care nursing team are looking for a Band 4 Assistant Practitioner to assist them in providing high quality care to patients entering the end of their lives. The role will include a mixture of patient and carer assessment, generic staff training around end of life care and liaison with treating teams around symptom management. Candidates should be strong patient advocates with good communication and clinical skills. The role involves elements of discharge planning and data management.
