Full Job Description

South Tyneside is looking forward to the next phase in our development of support for Primary Care Networks focusing on the Cancer pathway, and we are looking for enthusiastic, committed, can-do people to join our journey.

We have established and embedded a primary care cancer care coordination service for all patients in South Tyneside who are diagnosed with cancer. We are looking to increase our existing team, The Care Coordinator will work with the network in supporting them with the NHS England Early Cancer Diagnosis DES. A large part of your role will involve supporting patients in the early stages of their cancer diagnosis through treatment and living with cancer. The role is key to supporting patients after they have been diagnosed with Cancer and helping them to understand the implications of their diagnosis and what the next steps are, matching the support that is available locally to help patients during this worrying time.

Successful candidates will work with patients to complete non-clinical elements of cancer care reviews and help patients understand why cancer screening is key for early diagnosis and support them whilst awaiting diagnostic tests for possible cancer and investigate those patients who fail to respond or attend for this. You will act as the patients coordinator between hospitals, community and other services and organisations.

South Tyneside Health Collaboration (STHC) is a not-for-profit GP Federation and valued system partner, set up to bring together South Tyneside General Practices to work collaboratively in delivering services at scale across the borough of South Tyneside, reinvesting in primary care services and improving GP capacity.

STHC manages two of its own general practices, one in the beautiful coastal village of Whitburn and another, a developing training practice in central Jarrow, with a combined list size circa 8,500.

The role is for 25 hours per week on a fixed term basis for 12 months.The salary shown above is based on a full time role of 37.5 hours, therefore the actual salary pro rata to the 25 hours would equate to £16,848.50 per annum.
