Are you passionate about person centred care? We have an exciting opportunity for a support worker to join our Clinical Nurse Specialist Team as a Cancer Navigator. We have partnered with Macmillan to develop a cancer support navigator service. You will be supporting the Clinical Nurse Specialist in providing person centred care acting as a key point of contact for the patient, signposting to other services, providing information and facilitating holistic needs assessments., Respond appropriately to queries, take phone messages and pass on written and verbal information to patients
Provide relevant information to patients, family/visitors/carers and colleagues
Complete documentation accurately and contribute to reports of patients' activity and progress, recognising that the CNS has personal accountability for entries to patient records made by others under their supervision
Provide feedback on patient care and/or relevant patient communications to relevant healthcare professionals
Participate in discussions about patient care and clinical service development, including relevant team meetings
Maintain patient confidentiality

We are seeking an experienced Health Care Support Worker with excellent communication, problem solving and organisational skills.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.

Develop communication skills in order to convey complex and sensitive information effectively to patients, carers and other staff, overcoming any barriers to understanding and providing support during distressing or emotional events
Communicate with people in a form and manner that is consistent with their level of understanding and culture, and recognise barriers to effective communication

    NVQ 4 or 3 plus additional training to diploma level equivalent or relevant experience
  • To have knowledge of cancer related issues
