Full Job Description

We are recruiting for a Cancer Support Worker who shares our ambition and vision to deliver the Best of Care by the Best People and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff., The Cancer Support Worker's role (CSW) supports the coordination of care for cancer patients throughout their treatment pathway. The CSW works with the Macmillan Breast Clinical Nurse Specialists and the multidisciplinary team (MDT) to provide support, advice and assessment for patients following a diagnosis of breast cancer.

To contribute to our exciting future and become part of our team, apply today., Work cooperatively as part of the MDT, assisting nursing and other clinical staff to deliver a high standard of holistic patient care
Attend weekly MDT meetings, assisting the CNS with MDT preparation, subsequent tasks, referrals and administration
Assist in provision of MDT contact details for patients and guidance about who to contact for what purpose
Provide administrative support to CNS for specifically identified tasks associated with patient care, using IT systems as appropriate
Assist CNS team to provide helpline access and triage calls using agreed protocols
Provide general telephone advice and information, signposting patients/family & carers to further support appropriately. Make and assist with referrals to tertiary services
Undertake Holistic Needs Assessments (HNA), make appropriate referrals and develop care plans to support patients
Assist CNS team to encourage patients, their family and carers to make lifestyle choices that are best for themselves and their lives
Proactively identify needs and changes in management required through patient assessments and respond appropriately
Act as advocate and facilitator to highlight issues perceived as barriers to care
Communicate effectively and promptly with CNS team about concerns raised during HNA
Signpost patient, family members and carers to relevant services and support
Make and assist with referrals to tertiary services
Work with the Personalised Care and Support Facilitator to develop and host Health and Wellbeing events and attend tumour specific support groups.
