Full Job Description

The successful candidate will provide a consistent high quality specialist palliative care service with significant post registration experience including recent experience in palliative care you will need to demonstrate excellent communication and leadership skills and will need to be committed to developing practice.

Working collaboratively as part of a team and as as an autonomous practitioner and build effective relationships with colleagues across the MDT.

There are seven prisons in the North East, each one being slightly different, and four where 24 hour care is provided. The Macmillan Clinical Nurse Specialist will work in partnership with the prison healthcare teams and prison staff to ensure patients are offered the care and support they need.

This exciting 'hybrid' post will work as part of the Communuity SPC team and be based within this team.

This substantive Band 7 post offers an exciting opportunity for a suitably qualified and experienced Nurse to work within the Specialist Palliative Care in the Community setting., If you are being interviewed you must accept an interview slot in the system to continue, even if you have arranged with the manager

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
