Full Job Description

The Haematology-oncology nursing team and the Macmillan Centre are looking for an experienced and dynamic new colleague to join our growing services supporting the people of Croydon affected by cancer.

The purpose of the role will be to support patients through their cancer pathway and beyond; to navigate the complex health and social care system during this time. The new post holder will be responsible to the Haem-oncology CNS two days a week and one day to the Macmillan Centre and Personalised Care Manager.

As a Macmillan Cancer Patient Support Officer, you will have direct contact with people affected by cancer, offering emotional, practical and financial information and support.

You will work flexibly, being visible and accessible to those who access the services. You will provide support face to face, over the phone, via email, letter and text.

Join us and we will provide a comprehensive induction programme, including, training and insight visits. We also fully support long term access to both in-house and external training. Supervision is fully supported and in addition to this you will have regular support sessions with the team and your peers.

We recognise that this can be an emotionally challenging role and we actively encourage access to wellbeing support sessions., Macmillan Patient Support Officer - Band 4Part-time , 22.50 hours per week

The purpose of the role will be to support patients through their cancer pathway and beyond; to navigate the complex health and social care system during this time. Care Coordination is the joining up of services, coordination, information and communication between caregivers, treatment providers, those living with and beyond cancer and their families, that creates a seamless experience of care (NHS Improvements, 2011).The post holder will support both the Clinical Nurse Specialist (CNS) Team and the Macmillan Information & Support Centre Team in providing exemplary personalised cancer care.

The post holder will be responsible to the Haem-oncology CNS three days a week and two days to the Macmillan Centre and Personalised Care Manager.The post holder will be required to coordinate with appropriate services to maximise the delivery of the cancer pathways, thereby improving the quality and experience for patients and their carers.
