Full Job Description
Salford Royal is a regional major trauma centre providing acute major trauma services across the whole of Greater Manchester. In 2024 the service saw the opening of the Greater Manchester Major Trauma Hospital (GMMTH).
An exciting opportunity has arisen to undertake a Senior Major Trauma Co-ordinator role, as a secondment opportunity, to cover a secondment.
Applications are invited for a committed individual with a passion for major trauma care to join the Major Trauma Team and take the role as a Major Trauma rehabilitation Coordinator.
As part of the Major Trauma Team you will be responsible for the coordination of the major trauma patients care and the journey through the trust from the Emergency Department to discharge or rehabilitation.
The role will focus on facilitating patient flow in Major Trauma rehabilitation pathways and discharge planning., To provide specialist support in the assessment, planning and implementation of care for major trauma patients admitted to Salford Royal. This will include work across the organisation to coordinate thecare of all patients who require specialist rehabilitation. It would include coordinating the patients care whilst they are in the acute setting, identifying the appropriate pathway and facilitating the transfer to the appropriate facility.
The Rehabilitation Coordinator will work as an integral member of a highly skilled multidisciplinary team to coordinate the rehabilitation patient pathway from admission through to discharge. This will include all patients admitted to the Major Trauma Centre at Salford Royal identified as having complex rehabilitation needs.
They will be a representative for rehabilitation at Major Trauma Governance and associated forums locally at Salford Royal. They will also represent rehabilitation at Salford Royal at a Network andNational level. This will involve participating in various forums with potential to feedback data on any projects being completed to enhance patient care
Working with other members of the major trauma team you will ensure the completion of the rehabilitation assessments within the national guidelines time limits and the completion of the rehabilitation prescription. You will be accountable for your own professional actions and not directly supervised.
