Full Job Description

The post holder will work as an administrative member of the Maternal Medicine team to ensure all women booked for maternity care at Frimley Health NHS Foundation Trust (both Wexham Park/Frimley Park site) that require referral and care to the Maternal Medicine Consultant receive high quality antenatal care.
The post holder will play an essential part in ensuring any woman referred onto the Maternal Medicine Pathway is identified and can be followed up with the most appropriate care which will involve liaison/referral to the Maternal Medicine Centres for expertise.
Please note this post will need to provide cross site between Frimley and Wexham Park hospital when necessary, however no travel will be required., The post holder will undertake a wide range of administrative tasks to support with the success of the Maternal Medicine Pathway.
The post holder will be expected to liaise closely on a day to day basis with the lead consultants for Maternal Medicine both internally and at tertiary centres, the lead midwife for antenatal clinic, community midwives, allied departments, women on the maternal medicine pathway, other stakeholders and members of the public. There will be a close working relationship with the maternal medicine consultants and lead midwife for the antenatal clinic, the maternity ultrasound staff, the community midwives, specialist consultants, antenatal clerical staff.
The post holder will be responsible for completing and reviewing data and for ensuring communication to the multidisciplinary team about this cohort of high-risk ladies both internally and externally., + To provide administration support to the maternal medicine high risk pathway
+ To collate data on all women on maternal medicine pathway in excel format and ensure spreadsheets are completed within agreed timeframes and maintain high standards of record keeping in the electronic maternity records and manage excel spreadsheet data
+ To support the Maternal Medicine Obstetric consultant and Maternal Medicine service to coordinate multi disciplinary meetings with internal and external stakeholders
+ Attend meetings, taking minutes and distributing minutes
+ To prepare of care plans for patients based on MDT outcomes
+ To liaise with patients regarding appointments both at FHFT and with tertiary centres
+ Coordinate results and letters/information from internal and external sources/clinicians
+ Assist with audit and follow-ups
+ Link with GPs and other agencies when required and to communicate with the Maternal Medicine Lead
+ Assist with formulation of EPIC notes and documentation
+ Cover for the other FHFT site when counterpart absent
