Here in Oxfordshire, we offer a unique set up of our Maternity Services with around 7500 babies born annually. For those working with us, we have a substantial support network for staff, focusing on wellbeing and development. This includes a team of professional Midwifery Advocates, Wellbeing Champions, staff networks and a staff Maternity Psychologist. You will be supported by the Practice Development Team who provide regular study days and updates and support staff clinically. We genuinely believe that by supporting our staff to feel happy and offering support within their work environment, we ensure the best quality of care for the women and birthing people who access our services. The post holder aims to facilitate effective, efficient and timely discharge of women and babies from the hospital., To work as a member of the ward team and work with the wider multidisciplinary team to facilitate maternity discharges in a timely manner., This job is based on The postnatal ward, Level 5, of the Women's Centre . The Postnatal Ward is a busy environment with up to 42 beds including a transitional care unit. Every day, women and babies are discharged and the role of the discharge coordinator is to work alongside the Multi disciplinary team to ensure timely, accurate and safe discharge of patients from the postnatal ward.
- NVQ Level 2 or similar
- Care Certificate (or willingness to complete)
- Computer literate with good knowledge of Microsoft packages Desirable criteria
- Working knowledge of electronic patient record (EPR)
- NVQ Level 3, Understands the need for strict confidentiality
- Committed to a caring approach
- Previous experience in healthcare environment
- Experience working as part of a team Desirable criteria
- Evidence of collecting audit data
- Previous experience in customer liasion/interagency
- Experience assessing and making plan for patient discharge, Able to communicate effectively via different methods
- Able to carry out delegated tasks under supervision of nurses and midwives
- Good time management skills
- Good organisational skills
- Able to prioritise workload
- Able to deal with challenging situations and others emotions e.g. aggression/anger/bereaved, Experience of working in challenging and busy environment
- Flexible and adaptable
- Evidence of good attendance record
