An exciting opportunity has become available within the specialty of Community Care Coordination - the Single Point of Access for Adult Community Services.

We are seeking a Matron to lead and ensure delivery of safe and effective services. Working across community and acute services providing support in the development of robust safe pathways and ensuring patients have the right access to right services at the right time.

The role will offer an opportunity to be integral to the ever growing transformation and redesign of community services to meet the needs of our population and looking at new ways to support people to be cared for in the community. You will need to have a passion for avoiding unnecessary hospital admissions and a drive to work with stakeholders across the system to develop services.

You will need to demonstrate proven managerial and leadership skills, strategic thinking, and quality improvement experience. You will be working in an established leadership team within a Community Directorate and have access to development opportunities, including a supportive environment to help you harness your potential.

Main duties of the job

To lead one of the community services portfolios and further evolve the team in line with the NHS long term plan, winter planning, place workstreams and hospital avoidance agenda.

The Matron/ Clinical Lead will be expected to work collaboratively with multiple internal and external stakeholders to further enhance the service and team as a whole.

Ensure the team functions to the highest standard ensuring safe and high quality care is delivered in a responsive way.

Ensuring performance and governance is maintained effectively at all times.
