Full Job Description

Provide a visible, accessible, and authoritative presence to whom all staff associated with the medical wards they are working with. Responsible for delivery advice to the staff on the medical wards. In close collaboration with the Infection Prevention and Control Team, responsible for upholding the implementation of infection prevention and control standards. Maintain own clinical/professional expertise and act as an expert advisor within the Clinical Care Groups Work collaboratively with the Trust multi-disciplinary teams (MDTs) in improving the planning and delivery of all aspects of safe, effective, and timely patient flow. Work alongside the community services, AED and assessment areas and clinical teams to develop modern, patient centred pathways, which support improvements in the patient experience. Contribute during the daily Bed Management meetings ensuring that information is communicated/escalated directly to the Care Group Managers of the Day, clinical Teams, and Hospital on
call Managers. Contribute to the delivery and development of Clinical Governance relating to the care of older people to ensure that core standards are met and provide effective leadership for all members of the medical wards in relation to governance issues. Lead and co-ordinate risk assessments, identifying, planning, and evaluating remedial actions taken. Investigate and resolve any formal clinical complaints in conjunction with the Care Groups and Management Teams, responding fully, taking corrective action, implementing action plans, and monitoring for continuous improvement. Work with the Infection Prevention and Control Team to ensure prevention of hospital acquired infection/transmission; a key objective with the outcome of a reduction of hospital acquired infections, including CDT, VRE and MRSA Ensure compliance with Trust and departmental policies and procedures. Maintain Professional Registration. Adhere to relevant code of Practice of Professional body.
