Full Job Description
Working in a multi disciplinary team your work will help support the management of both inpatient and outpatient care., + To use specialist knowledge of the patient pathway to proactively influence the tracking records using high levels of communication to expedite appointments/procedures as necessary and to resolve issues which could lead to target failures.
+ To persistently track, update and upload all the data referring to the MDT referrals and update the electronic Tracking Records, Specialist or local MDT Teams and local Services team accordingly.
+ Provide full assistance to the clinical team in the co-ordination of the Multi-Disciplinary Team Meetings and representing the MDT Teams to disseminate the information regarding the patient pathway.
+ Communicate effectively with all service teams, Senior Clinicians and other Trust Senior Managers. Provide support to the MDT meetings, including recording actions and outcomes and national data collection, and other data collection identified by the MDT.
