Full Job Description

An opportunity has arisen in the Performance Management Department for an MDT Coordinator/Patient Pathway Tracker. The team requires an enthusiastic team member who is committed to having a positive impact on cancer patient pathways. You will have had experience working within the NHS preferably within a cancer data environment. An understanding of current cancer pathways and MDT processes would be extremely desirable. You will be required to work flexibly to meet the needs of the service., As an MDT Co-Ordinator/Patient Pathway Tracker you will be responsible for completing, but not limited to, tasks such as:

Tracking target patients in a timely manner from referral to first treatment. Attending regular meetings and provide updates on the patient pathway. Accurately maintain a patient tracking list. Liaise with other trusts.

Being responsible for the facilitation of Multi-disciplinary team meetings ensuring MDT meetings run effectively and are well structured.

Accurately prepare and circulate lists of patients for discussion at each multi-disciplinary team meeting (MDT) using information provided by secretaries, lead clinicians and nurse specialists.

To attend the multi-disciplinary team meetings, keeping comprehensive records of attendance and outcomes of the meeting, and distributing to relevant team members. To ensure timely and accurate input of the outcomes of the MDT meetings onto local patient administration system, CWP.

Candidates must have Microsoft Office Suite experience. You will have good organisational skills and be able to communicate confidently with all levels of staff on a wide range of information
