Full Job Description

Vida Healthcare is currently looking to recruit a reliable and hardworking person to join our busy team. This position will be based predominantly in the King's Lynn area, however multi-site working will be expected occasionally. This position is for 37.5 hours per week and you will work under the recalls and results team leader.,

  • Ensure that documents, forms and requests received are logged and dealt with accordingly and within agreed timescales.
  • On occasion, or where concern arises, contact patients to verify their consent and their understanding of what has been consented to, keeping a record of this auditing/verification.
  • Prepare forms and documents in response to appropriate requests and ensure that these are only sent via appropriate and designated mechanisms.
  • Liaise with GPs in relation to replies, and suitability of responses, for medical information requests.
  • Ensure that the electronic patient record is updated with all patient related activity and documentation.
  • Work with, and alongside, the summarising team regarding tidying and re-summarising medical records to ensure accuracy and/or quality.
  • Check all information prior to release for third party data or any other items that may require redacting and ensure that this is acted upon.
  • Seek support and guidance from the team leader, Caldicott Guardian, IG lead and DPO as required.
  • Assist in supporting the management and integration of incoming medical records.
  • Understand and act upon clinical information received to advise patients of their results according to clinician instruction.
  • Manage queries from patients regarding their information requests and upcoming tests or results.
  • Process district nurse referrals for all sites.
  • Support the proactive management of patients to reduce number of patient contacts, ensuring all annual/monthly checks are completed in a 'one stop shop' approach.
  • Process tasks and workflows from nurses and other practice staff to arrange appropriate monitoring of patients.
  • Identifying potential system and process improvements within the practice and the wider healthcare system that support patients getting the best outcomes in terms of their care.
  • Ensure that patients on the health registers are contacted, offered, and followed up through the annual monitoring requirement for these groups. Ensuring that information is coded and reported to meet targets. Escalating issues affecting this achievement.
  • General:
  • Be able to work unsupervised whilst also being an effective team member, communicating and supporting colleagues in achieving the practice aims.
  • Liaise and communicate with wider practice staff and participate in meetings as required.
  • Attend relevant training to meet the requirements of the role and improve skills.
  • Maintain data security at all times (clear desk procedures, lock out computers etc.).
  • Be aware of information governance standards and policies and individual's rights under GDPR and Data Protection.
  • Work with attention to detail and be able to identify areas for improvement.
