Full Job Description

An exciting opportunity for a proactive and forward thinking pharmacy technician to join the BNSSG ICB Medicines Optimisation team has arisen.
The post holder will be a registered Pharmacy Technician and will enjoy a varied role within a dynamic, friendly and well respected BNSSG ICB Medicines Optimisation team.
The role will include reviewing current medicines optimisation services and processes, facilitating optimal pharmaceutical care in BNSSG for patients to improve safety, drive efficiencies and reduce waste. This will ensure high quality, safe and cost-effective medicines optimisation in BNSSG. The post holder will be support the ICB and key stakeholders to deliver the medicines optimisation work programme and will have the opportunity to engage in system-wide working to improve medicines optimisation for our population., · To support the medicines optimisation team to deliver all aspects of the medicines optimisation work programme, including quality and safety and cost effectiveness.
· To support medicines optimisation pharmacists embedded in General Practice with the work to promote the safe, effective and cost-efficient use of medicines within primary care such as with the prescribing quality scheme and the cost saving dashboard.
· To contribute to the medicines safety agenda by supporting the incident reporting system and any system-wide work to prevent medicines related incidents.
· To take a lead on projects focussed on sustainability within medicines optimisation including reducing medicines waste.
· To support with data extraction and business intelligence functions to support the work of the team., · Support the medicine optimisation team to ensure that the portfolio of tasks/projects is planned, managed and delivered effectively.
· Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally.
· Contribute to the information management of performance, taking a lead for specific medicine optimisation projects
· To take a lead on projects focussed on sustainability within medicines optimisation including reducing medicines waste.

· To lead some and support other senior colleagues, to review of current medicines management services for people in the community and identify the need for provision of new services and identify areas of potential service improvement and cost savings.
· Deliver against organisational objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines
· Support training and induction of new staff.
· Lead on development, implementation, monitoring and evaluation of new information systems/databases as required
· Actively supports and contributes to the development of key performance indicators for the successful assessment of performance
· To lead on the development appropriate guidelines promoting the most appropriate and cost effective use of medicines,
· To liaise with health and social care professionals on the provision of medication review and pharmaceutical care
· To complete projects ensuring patient safety in relation to medicines
· To provide appropriate and current advice and promote good clinical and cost-effective practice
· To participate in risk management work relating to prescribing and ensure compliance with medicines legislation
· Provide pharmaceutical technical advice, support and information to healthcare professionals in BNSSG
· To review and facilitate the development of medicines management services for people in BNSSG liaising with local healthcare teams and social care organisations
· To liaise with other ICB colleagues including the quality team and primary care stakeholders to review and monitor the provision of services in primary care
· To work in partnership with community pharmacy providers to implement agreed action plans to ensure improved medicines management processes
· To lead on work streams to prevent medication related incidents and harms to residents in collaboration with other relevant stakeholders.
· To liaise with health professionals to address medicines optimisation issues across the interface relating to hospital admission and discharge
· To develop, manage and provide training to other healthcare professionals and other care home staff; liaising with other education providers where necessary
· Analyse, interpret and present data to highlight issues, risks and support decision making
· To represent the ICB on various meetings including multi-agency groups and input into other strategies relating to medicines optimisation
· To work within the professional standards established by the General Pharmaceutical Council
· To liaise with other ICBs on medicines optimisation projects to deliver ICB objectives
· Communicates complex information to a variety of healthcare professionals
· Communicates with member practices of the ICB on medicines management issues in care homes
· Works collaboratively with other members of ICB staff
· Contributes to the production of medicines optimisation newsletter for BNSSG
· Provides education sessions on medicines-related issues in care homes and liaises with a range of providers where appropriate.
· To facilitate the commissioning of education and training to primary care staff.
· Monitor and process financial claims
·Promote cost effective prescribing in primary care, including the monitoring of prescribing costs and associated savings.
+ Designs and carries out relevant audits and takes forward findings, completing audit cycle.
