Full Job Description

Hospital at Home promotes harmonisation of working across the acute hospitals and the community services within MFT, utilising the skills and resources available to successfully manage the acute care of patients whilst they remain in their own homes. We are looking for a General Pharmaceutical Council (GPhC) registered pharmacy technician with substantial experience of working within primary or secondary care. You will work alongside the pharmacist as part of a multidisciplinary team to provide medicines support and positively contribute to patient care. You should be confident in the ability to work independently with excellent organisational and interpersonal skills.

The successful candidate will become an integral part of the well supported and award-winning medicines optimisation team in providing a patient facing pharmaceutical service, working closely with the wider pharmacy teams and other services across MFT.

The Pharmacy Technician is responsible for providing technical input as part of the pharmaceutical support to the Hospital at Home Service. Domiciliary visits to patients for medicines optimisation interventions is a key feature of this role as well as multidisciplinary team (MDT) working. Other areas include:

Support the delivery of medicines optimisation to the Hospital at Home Service

Support pharmacists to establish safe working arrangements for medicines in the Community Medicines Optimisation Service

Assist in the development and implementation of pharmaceutical care plans

Liaise with pharmacists, technicians and other health care professionals in secondary and primary care to ensure continuity of care on patient transfer.

Carry out medication reviews with patients in area of clinical competence

Contribute to the discharge planning process

Identify and act on medication errors/queries within area of competence, reporting complex issues to a medicines optimisation pharmacist as appropriate.

Assist in the management of prescribing across primary care

Provide support to other CMOS workstreams as necessary.
