To provide expert pharmaceutical technical support to the medicines optimisation operational team in a designated location to help support the implementation of the medicines optimisation work programme To promote high quality, cost-effective prescribing and best practice for patients within social care settings, or in their own home via domiciliary visits or in GP surgeries. To implement policies, procedures and initiatives relating to medicines management/optimisation within care homes, audit and evaluate the implementation of change and produce high quality reports
