Full Job Description

Memory Assessment Nurses are responsible for assessing service users for signs of organic progressive disease (dementia). The findings of assessment are discussed with the wider MDT with regards to a potential diagnosis (including appropriate plan regarding this) and a supportive care plan for the service user and their carer/family. As part of the pathway and nursing duties, a Band 5 MAS nurse would offer post diagnostic support, which includes practical information, signposting and emotional support after diagnosis., The successful applicant will be expected to demonstrate the capacity to work independently but under the supervision of senior team members. You will receive clinical supervision / mentorship and a robust personal development plan to support and develop your practice.
It is desirable that you can demonstrate experience and knowledge with memory assessments and evidence based tools used for diagnosis. You will be able to demonstrate an in depth understanding of the needs of service users with dementia and have an ability to support service users and their families/carers practically and emotionally post diagnosis.
MAS Nurses undertake their work in clinics and home visits. There is flexibility allowed in remote working for documentation and administrative tasks. It is essential that you have a full UK driving licence and use of a car. Successful applicants will be offered a comprehensive continuing professional development package which will include access to appraisal, clinical and managerial supervision.
Service operating hours are Monday-Friday 9am-5pm

We are currently looking for a part time Band 5 Memory Nurse in the East/North East locality for 30 hours a week, working days are negotiable over Monday-Friday. This locality is based near Chapeltown in Leeds LS7. We are looking for a committed and motivated practitioner who wants to consolidate their nursing practice in memory services and develop their skills, experience and knowledge.
