Full Job Description
This exciting post will be working in the Memory Assessment Service (MAS) providing assessment, potential diagnosis and signposting to other services and support networks for individuals aged 65 and over referred to the service with cognitive impairment.,
- To work as part of the multi disciplinary team to screen for appropriate referrals
- To complete an assessment of clinical presentation and symptoms
- To share information amongst the multi disciplinary team and create appropriate and person centered care plans
- To refer for appropriate diagnostic procedures eg scans, neuropsychology, occupational therapy.
- To make independent diagnostic decisions based on consultation, history, presentation, pathology and/ or imaging results and to seek supervision from service medical support in complex diagnostic situations.
- To complete accurate records
