Full Job Description
The post holder will carry out detailed memory assessments, provide clear presentation in MDT discussions, support diagnosis formulation, provide feedback to patient and families, carry out medication reviews and support the post diagnostic support offered within the service.

To demonstrate understanding and use of clinical supervision to reflect upon and modify own and others practice.
To ensure that own practice remains within legal, ethical and professional parameter and is open to scrutiny from peers
Must hold a current, valid first level registration with the Nurses and Midwifes Council.
Management of caseload- clinical
Provide leadership, Mentorship and supervision for support workers, Band 5 nurses and student nurses, as appropriate.
Good Computer literacy skills
Management responsibilities:
Management of clinical caseload
To exercise good personal time management, punctuality and consistent reliable attendance.
To participate in the team across the Memory Service.
To supervise and support staff at Band 6 and under grades.
To develop, implement and evaluate policies and strategies relevant for the Memory Service.
To demonstrate the ability to effectively use available resources.
To co-ordinate day to day activities of caseload
To analyse and respond to clinical situations.
To work within the Memory Operational Policy
To maintain risk assessments for clinical caseload., To hold professional responsibility for assessing individual care needs, risk assessments, development, implementation and evaluation of care plans, in line with Oxleas Foundation Trust CPA policy. With particular focus on service users with Dementia.

Clinical:
To accept care co-ordinator responsibility for a clinical caseload.
To initiate assessments and treatment programmes in service users homes and at the memory clinics.
To facilitate the inclusion of social care services in the care plan in line with assessed need, agreed eligibility criteria and agreed procedures.
Respond to the needs of people in an honest, non judgemental and open manner, which respects the rights of individuals and groups.
Actively engage with service users and their carers in the provision of holistic, needs-led care, which takes account of the physical, psychological, emotional, social and spiritual needs of individuals.
Have an understanding of the needs and rights of carers and undertake Carers Assessments with a view to offering supportive services.
To maintain accurate RIO records, including core and risk assessments, care plans, progress notes and contacts as required.
Have a good understanding of the Mental Health Act 1983 and the Mental Capacity Act 2007. Undertake mental capacity assessments and have an understanding of their responsibilities under both Acts.
Have an understanding and knowledge of the Care Act 2014 and its implications to practise.
Demonstrate an understanding of Safeguarding Adults policy and procedures.
Ensure that service users and their carers are up to date in their knowledge including having a copy of their care plan, information about treatment and the role of the care co-ordinator.
Assist patients with social and practical activities, such as education, leisure and financial management and assisting in developing and maintaining personal and social networks with families, carers, and others in line with the social inclusion model of care.
Advise and assist patients in maintaining daily living skills such as personal hygiene, self-care skills, social skills, and nutritional management.
Implement evidence based practice in all aspects of clinical care, developing an ongoing awareness of significant nursing and other mental health research relevant to the care group.
Develop effective working relationships with patients, families and
carers by facilitating engagement and therapeutic co-operation through
the use of flexible and responsive strategies.
Facilitate groups within the community for clients, carers and relatives.
To provide advice, education and work in co-partnership with residential/nursing homes.
Be responsible for the administration, carriage and storage of medication in accordance with relevant policies and clinical procedures. To maintain drugs records as agreed policy.
To act as a resource within the CMHT, giving advice and offering assistance as appropriate [e.g. joint visits]
Demonstrate a commitment to equal opportunities and an understanding of the impact of social and cultural diversity on service users' and their carers' experiences of mental illness and mental health services.
Research:
Participate in any research, audit, Clinical Governance or Clinical Effectiveness projects, as required.
Demonstrating ability to initiate and maintain positive change within own and other areas.
Communication:
To communicate effectively within the multidisciplinary team and with service users and carers on a range of issues that are often complex in their nature and in complex situations.
Participate in supervision and Professional Development Reviews and demonstrate reflective practice through the Knowledge and Skills Framework.
To maintain electronic and written clinical records ensuring confidentiality is maintained at all times.
Provide supervision, mentorship and leadership for junior staff within the Memory Service & Student Nurses, appropriate.

We are looking for a registered mental health nurse with experience in Memory services and or Dementia care. The post holder will work as part of a well established memory service, carrying out assessments, diagnostic appointments, follow up appointments, group work and carry a caseload.
The post holder will need to be motivated, passionate about Dementia care and be prepared to work as part of a team that are striving for excellency. They will need excellent communication skills, the ability to be flexible, work proactively and work well as part of a team.
