If so, we have an exciting opportunity for you to work in our Memory Assessment Service, based at St. Mary's Hospital. We are looking for an enthusiastic person with a passion for delivering excellent care to join our team. The team aims to proactively provide assessment, potential diagnosis and signposting to other services and support networks for individuals aged 65 and over referred to the service with cognitive impairment. We work closely with care facilities, people and families in their own homes, GP liaison and Adult Social Care and our NHS Dementia Service to prevent admissions.Main duties, tasks & skills required To work as part of Mental Health Services frontline response to people referred for support and/or treatment. The post holder will risk assess, complete inter-referrals as needed, and plan intensive in-reach support where appropriate. The post holder will carry registration as either a Registered Nursing Associate, Care Certificate level 3 or equivalent. Duties include:

  • To ensure and take responsibility for the provision of comprehensive assessments of needs and risks of people referred to the service.
  • To take responsibility for planning and the delivery of care to people requiring an acute and intensive response from this service.
  • To plan and take responsibility in collaborating with service users and their carers together with other services/agencies on how people's needs are going to be met in short-term and long-term situations.
  • To be active in the development of the service and continued development of professional practice and role.
  • To ensure that relationships and own behaviour are managed in line with the Trust's Equalities Framework competencies.
  • To ensure all communications are undertaken in a sensitive, confidential and professional manner.
  • Interdisciplinary working and liaison with internal and external agencies to promote holistic and person-centred care and inclusion, We are happy to talk Flexible Working - all requests for flexible working options can be discussed as part of the interview process.

    Registered Nursing Associate, Care Certificate Level 3 or equivalent.
  • Current national legislative and professional requirements relating to social and health care and processes implemented locally (Community Care Act, Mental Health Act), Evidence based and experience of reflective practice. Ongoing assessment of risk, care intervention and management. Experience in therapeutic engagement to patients in groups. Wide knowledge/ appreciation of a range of models of practice in social and health care. Knowledge of interventions and strategies which embrace social and health care needs. Risk assessment and management. Engagement Solution Focused working Liaison/negotiation Decision making, Ability to use assessment skills and tools effectively. Good verbal and written communication skills To undertake mandatory training To undertake lone working to patients with mental health concerns. Car driver Clean driving licence Ability to work under pressure. Good communication skills. Good therapeutic skills including empathy, active listening and understanding of therapeutic boundaries.
