The interventions will be focussed on assisting the person to identify and achieve their agreed goals of recovery. These will include interventions related to; meaningful occupation, accessing employment, assessing and promoting physical health, social inclusion, activities of daily living and mental health promotion. The post holder will also be involved in supporting the service user to complete appropriate outcome measure tools and participate in reviews

of the service users care and effectiveness of the interventions provided. Things that you will have: Advanced Apprenticeship in (Mental) Health and Social Care or NVQ Level 3 in (Mental) Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSEs or functional skills) Experience of working with people with Mental Health Needs non-Dementia related Strong team-working skills A high level of self-motivation and a flexible approach A positive attitude and be naturally caring and compassionate A commitment to high-quality care and values and evidence-based practice Excellent teamwork skills, and professionalism Please note you will need to travel to service users houses, different MPFT bases and so you will need access to the appropriate form of travel.
