Full Job Description

Care Home Liaison Team (CHLT) are looking for enthusiastic and experienced mental health nurses to join the team. CHLT is a community based mental health team that works with older adults (65+) with mental health issues in care homes across Cardiff and the Vale. Our service provides assessment and advice on the management of resident's mental health needs. We provide care staff with training and support in managing their resident's mental health needs. We also liaise closely with GP's and other community teams to ensure holistic care of the resident is provided.

CHLT is a multi-disciplinary team of nurses, doctors, support workers, pharmacists and other health care professionals. Working collaboratively to maintain the standard of mental health care across care homes. We have members of the team that are trained in Positive Approaches to Care and Dementia Care Mapping. This helps the team provide robust assessments, interventions and advice/education to support care home staff in their work with people living with Dementia.

Training in psychological interventions to support people living with mental health issues other than dementia is also available. We encourage the development of our staff and the successful candidate will receive training and will be supported to attend relevant study leave to help them fulfil this role.

To devise, implement and follow up specialist treatments across the disease spectrum for patients within a specific group. To be a skilled practitioner providing clinical care using an extensive theoretical knowledge base and practical experience to underpin the role, while acting as a role model for the delivery of care.

To deliver specialist educational programmes to a range of healthcare professionals in relation to the specific disease including medical, nursing and Allied Health Professional staff.

The ability to speak Welsh is desirable for this role; Welsh and/or English speakers are equally welcome to apply.
