Full Job Description

The post holder will support the Council's statutory duties in relation to Adult Social Care, ensuring that the principles of 'Prevent, Reduce & Delay' are applied at all times including a focus on strength-based approaches to assessment and support.

The role will manage the day-to-day operational requirements of Mental Health Enablement and will be responsible for managing and coordinating a team of Mental Health Support Workers providing support to people within the community.

Under the direction of a Mental Health Team Manager, the post holder will respond to referrals to Mental Health Enablement. The role will work with other partners/professionals to assess and develop and/or oversee the development of quality care plans and ensure these are delivered to a high standard. The post holder will ensure that all appropriate protocols are adhered to, including coordinating caseloads, case allocations and managing risk.
