Full Job Description

Following investment, the Mental Health Liaison Service an exciting opportunity has arisen for a motivated and enthusiastic clinician to work within the dynamic Mental Health Liaison Team based at the Stonebow Unit, Hereford. The role will serve to identify and support patients who present within the acute hospital with a variety of mental health conditions alongside complex physical comorbidities. The clinical content of liaison psychiatry practice is complex, and every day brings a new challenge. We are seeking adaptable, flexible, and competent mental health practitioners who wish to develop their skills within the hospital setting. The hours of work are 37.5 comprising of 3/4 12Hr shifts . If you would like to discuss informally if this is the right role for you, get in touch with Recruiting Manager which can be found on the top right-hand side of the advert., It's a little unpredictable, as you can be called at any time to a crisis situation, we discuss referrals each morning and twice weekly we review the caseloads with our consultant psychiatrist. We complete comprehensive risk assessments for every patient, these can be completed independently although sometimes we discuss more complex patients as a Multi-disciplinary team. The team works hard to provide a fully integrated service for our patients, so it's not just about making sure their needs are catered for while they are in the hospital, but also when they leave. We talk to families and carers, GPs, community mental health teams, social care teams, community nurses, physiotherapists and voluntary organisations, and anyone who is necessary to ensure a full care plan is in place for a patient when they leave hospital, so the support they need at home can start the minute they arrive.
