Full Job Description

The Psychological Medicine Service in Stockport was established in 2017 to improve the psychological care of people with complex physical and mental health needs. The service has been responsible for development of services which have had some national recognition and a highly positive local evaluation.

'Psychological Medicine in Primary Care' (PMPC) is a well established team in Stockport The appointee will be part of an innovative team leading service developments alongside local health and social care providers.

This role will be an exciting challenge for mental health nurses, occupational therapists or social workers who are keen to work with patients with complex mental and physical healthcare needs. The successful person will be working across Stockport, working with colleagues from psychiatry and psychology. The post holder will be vital to the delivery of whole patient care.

Main duties of the job

The postholder will carry a relatively small caseload, and will liaise closely with other healthcare professionals and colleagues to provide effective joined up management of patient care through education, consultation, assessment and joint working of cases. The primary focus of this team will be patients with persistent physical symptoms and patients with complex mental health needs which are affecting their presentation to health and social care teams.

The post holder will act as a representative of mental health services within an identified neighbourhood multi-disciplinary team, providing specialist advice and support in relation to colleague interactions with patients.

Working within the team, they will advise on suitability of new referrals, assessments, offering interventions for adults with complex mental health presentations and complex comorbid mental and physical health.

The post holder will be a qualified mental health nurse, OT or social worker with demonstrable experience in working in a mental health setting. She or he will have specialist knowledge of working with patients with complex mental health needs or with patients with complex comorbid physical and mental health problems, and will ideally have an understanding of different psychological therapies and how these can benefit patients. Experience of MDT working and working in a primary care setting is also essential to this role.
