Full Job Description
We are offering a permanent role to work alongside our current nursing team and be part of the multi disciplinary team that really delivers. This is a unique opportunity to be part of a new initiative that is one of its kind in the country. Our vision and values are to work closely with our patient(s) using a 'hand on approach' and supporting them to a successful discharge into the community where they can live as independent life as they envision., As named nurse you be responsible for ensuring completion of collaborative care plans, updating relevant paperwork.
Medication management-
Participation in multi-disciplinary team meetings
being an advocate for our patient(s)
Facilitation of groups/Activities on the ward, As a staff nurse on the Wolds you will be allocated a caseload of named service users, as well as associate service users. There is also the expectation that you remain up to date of the care and treatment for all admitted onto the Wolds.
As a named nurse you will be responsible for the duties this role entails, such as personalised care plans that are tailored to the individual, written with them and not for them and relevant paperwork that helps aid recovery.
You will also have the opportunity to facilitate groups specific to our clients needs. Medicine management, Participation in MDT's, working closely with our Social Inclusion Workers for the best outcome for our patients through their reablement pathway.
As the Wolds is a 24/7 service we have to ensure safe staffing levels at all times. The working shifts within rehabilitation service are long days with a rotation of nights.
LPFT has been nominated as a good place to work, we have also been rated as Good by the Care Quality Commission (CQC) as well as an AIMS accredited service. There is the opportunity to advance your skills and knowledge through online learning, courses open to NHS staff and self learning.
You will also have monthly managerial as well as clinical supervision as we understand the importance of being able to reflect on our self practice, annual appraisals to identify the areas you wish to progress in to further your career.

The post is full time working hours on a rota system, Rotational nights, weekend, and bank holiday working- all of which attract shift enhancements. Excellent rates of pay as per agenda for change.
