Full Job Description

An exciting opportunity has arisen within the ICB's Complex Care Directorate for a motivated and experienced Mental Health Practitioner. The post holder will be either a Registered Nurse - Mental Health registered with the nursing and Midwifery Council (NMC) or AHPqualification or Qualified social worker.

The Post Holder will lead on adult mental health reviews and have case oversight forpatients with individually funded care including Section 117, Health funded and Long-Term High Dependency Inpatient cases, preventing delayed transfer of care and reducing out of area placements. They will manage a defined caseload of adults with mental health needs, including Older Peoples Mental Health.

The post holder will work in partnership with a range of agencies to ensure that individuals receive packages of care that meet and maintain their identified health needs. They will liaise with care coordinators/social workers and other members of the multi-disciplinary teamregarding individuals who have joint health and social care needs and who require specialist health input.

To note this post is a Part Time post (0.5 WTE)

Main duties of the job

Care planning and care management for health funded and joint funded cases, with oversight from the Complex care practitioner.

Responsible for the case management of patients with mental health needs who are eligible for NHS funding or who are likely to become eligible for funding.

Regularly review individual's care needs and adjust care plans to meet those needs ensuring that the patient, their carers/ family are included in the monitoring and review process.
