Full Job Description

To operate flexibly and carry out the below duties on a responsive, county-wide basis, potentially working across multiple teams according to the needs of service user and services.

To provide assessment and care planned interventions to clients on the caseload of the AIST service and to offer advice and consultation to other professionals as appropriate.

To hold care co-ordination for a named group of clients as per Autism Intensive Support Team ( AIST) criteria.

To act as lead practitioner for a named group of clients as per AIST criteria

To contribute to the development of this new service and ensure best practice care delivery as per NICE guidelines, This post is a countywide position, the teams main base being in the west of the County at Truro. The position may predominantly work with clients who are living in the west of the County, however this can be discussed further as appropriate
