Full Job Description

We are looking to recruit a community mental health nurse or Mental health practitioner or Occupational Therapist who has a special interest in Dual Diagnosis for the Mendip Neighbourhood Older persons mental health service, 3 days a weekThe successful applicant will be providing assessment and treatment for individuals who have severe and enduring mental health problems, this could also be alongside coexisting substance misuse disorders. The applicant can be based either in Chantry Day hospital in Frome or Willow Bank, Priory health park, Wells.All training and support will be provided and would suit someone who is looking to develop from a band 5 to a band 6 We are happy to discuss flexible working requirements., The postholder will work as a key worker within the Mendip locality ensuring the provision of a high quality service, working with individuals over the age of 65 years supporting with moderate to severe mental health difficultiesTo act as a keyworker for a caseload of people with mental health difficulties and developing their care plan, coordinating and reviewing their care and treatment outlined in the plan, at times offering direct individual and group interventions to these and other patients where appropriateThe postholder will carry a clinical caseload and will provide supervision and advice to other mental health staff in relation to their working practice with this service user groupThe post holder will promote awareness and best practice, actively supporting positive attitudes and a consistent approach towards assessment and treatment of this service user group using evidenced based interventions and treatment frameworksTo participate in wider multidisciplinary working across
primary and secondary care servicesTo promote health through provision of health education, information and advice
