Full Job Description

The Extended Hope assessment and support service aims to support young people facing emotional and mental health crisis outside of normal office hours or requiring, following assessment, a respite/crisis bed. It is staffed by both Residential Workers and Mental Health Practitioners who will undertake assessments of young people and can offer support to them and their family/carer. The service operates between the hours of 5pm -11 pm, seven days a week. Extended Hope has two main services A two bedded house in Redhill, Surrey where young people can go to be assessed and supported in a safe environment for a maximum of seven days. As well as providing respite during a crisis, Extended Hope and its staff also support the family to create a plan of care, hopefully preventing the situation escalating and a hospital referral. Out-of-hours emergency support - this urgent support service is provided by telephone 5pm - 11pm, seven days a week. This service is maintained by Mental Health Practitioners who can give advice/guidance and care when most 'day services' are closed. This role is that of a Mental Health Practitioner within the service and would suit Nurses, Social Care Professionals or Occupational Therapists with the requisite experience.

  • Working closely with Mindworks services during office hours, responding to emergency referrals of young people presenting in crisis.
  • As an autonomous practitioner, undertaking clinical assessment of children and young people who may have complex co-morbid mental health and/or social care needs and present in escalating crisis. You will be working alongside medical, nursing, social care and other professional colleagues to ensure that the holistic approach to meeting the needs of the young person and family/carer is considered.
  • Ensure that outcome measures for the service are implemented and audited.
  • Report to and advise referrers on assessment and treatment outcomes.
  • Support the Service Manager in maintaining the e-roster system for staff shift patterns.
  • Supervise other professionals of a lower grade and offer support and supervision to junior team members. For Extended Hope this will mainly be for the Support Workers who are caring for young People in the Ext Hope House for respite this can be through sessions on medicine management, risk management and informal supervision / training .
  • Leading on psychotherapeutic input with any Young Person admitted to Hope House alongside the team(s) who are working with them in the community - either Health and/or Social Care
