Full Job Description

as part of their appraisal process, and regular 1:1 supervision is provided. The post holder may also like to become actively involved in potential service development projects as part of their role.The team operates a 24/7 service, therefore there will be a requirement to cover shift work.,

  • The post holder will undertake collaborative care planning with service users and their carers.
  • Undertake risk assessment and record risks in line with the trust policy
  • Undertake shift co-ordination when allocated to do so.
  • Participate in Multi-disciplinary Team(MDT) and agree plans with patients in relation to their care.
  • Maintain their knowledge of evidence based home treatment interventions and deliver these interventions in line with service use care.
  • Undertake collaborative care planning with services users and their carers.
  • Undertake gatekeeping assessments to ensure service users are treated in the least restrictive environment.
  • Undertake allocation of shift co-ordination.
